Underneath the field where you put your keywords there is a field called COMMENTS there you can put in the credits. I always do it like this "made by <lj user="xyz">" SO everyone knows who made it and can go to the LJ to look for more.

Normally 100x100 icons go there just fine. And you don't need to credit them there.
